Step I

Upon receipt of your information, our office will contact you for a general one-on-one telephone consultation with one of our physicians.Step III
Based on the results from your one-on-one telephone consultation, if a treatment can be provided, you will be required to fill up, or download and mail to (Artox Holdings, Inc., 954 Willow Garden Court, Lake Mary, FL 32746), the MEDICAL HISTORY FORM and the MEDICAL RECORDS RELEASE FORM.Step IV
Upon receipt of Phase I payment, we will order and ship your DNA Kits. Follow the instructions on and mail back to the address listed on the box. Dr. Rosenberg’s office will send the request for the patient’s medical file, unless you notify your personal physician(s) to copy and mail your medical records to the address listed on the MEDICAL RECORDS RELEASE FORM. All this will take approximately 10 days. When all the reports from the DNA tests and the medical files are in, we will make the travel arrangements for your trip to Delray Beach, Florida.Step V
Our concierge desk will prepared your schedule, airline reservations, limo information, hotel information and directions to the Medical Office in Delray Beach, Florida. The full itinerary and schedule will be mailed to your physical address.Forms
